Two Step Process

You can order a Prince Edward Island Birth, Marriage or Death Certificate by following these easy steps:

Print out, complete and sign the appropriate request form
Send your completed and signed form by either
a) Fax or
b) Scan & Email, or
c) Mail

You will receive your certificate by mail or courier.
