Who May Apply For a Birth Certificate?

  1. You may apply for yourself;
  2. A parent named on the record of the child if the child is 18 years of age or younger; or
  3. A Legal Guardian if the person whose birth is registered is 18 years or younger. A Court Order that specifies the custody agreement between the subject and the applicant or a Separation Agreement that specifies the custody agreement between the subject and the applicant will be required.

Types of Birth Certificates


Short Form

  • Contains the individual's name, date and place of birth, sex, date of issue, registration date and registration number.

Long Form (Certified Birth Certificate)

  • Contains the individual's name, date and place of birth, sex, parents' names and places of birth, date of issue, registration date and registration number.

Fees

  • Short Form - $75.00 per certificate.
  • Long Form - $80.00 per certificate.
  • Rush Service – A surcharge applies if you wish to have the certificate delivered via courrier or on a rush basis.
